AVISO PARA EMPLEADOS COMMONWEALTH DE MASSACHUSETTS DEPARTAMENTO DE ACCIDENTES INDUSTRIALES SI USTED TIENE UN ACCIDENTE EN EL TRABAJO: • Avise inmediatamente a su empleador que tuvo un accidente. Contacto de RH del Empleador/Indemnización por Accidente Laboral Número de Teléfono • Avise al proveedor médico que usted tuvo un accidente en el trabajo y proporcione la siguiente información: Compañía de Seguros Dirección Empleador Dirección Número de Teléfono • Si el empleador no informa el accidente a la compañía de seguros, el trabajador puede presentar un Reclamo del Empleado (Form. 110). • Puede contactar al Departamento de Accidentes Industriales a través del 617.727.4900 o www.mass.gov/dia para obtener información adicional sobre sus derechos y elegibilidad para los beneficios según la ley de Indemnización por Accidentes de Trabajo. SI SE REQUIERE TRATAMIENTO MÉDICO: Los trabajadores que resulten lesionados pueden elegir sus propios proveedores médicos. Los costos por tratamientos médicos que sean razonables, necesarios y relativos al accidente laboral serán pagados por la compañía aseguradora que se menciona anteriormente. LOS EMPLEADORES NO PODRÁN TOMAR REPRESALIAS, DISCRIMINAR (SEGÚN LO ESTABLECIDO PARA CUALQUIERA DE LAS LEYES ESTATALES O FEDERALES APLICABLES, INCLUYENDO LA CONDICIÓN MIGRATORIA), NI BRINDAR INFORMACIÓN FALSA SOBRE EL PROCESO DE INDEMNIZACION POR ACCIDENTE LABORAL A SUS EMPLEADOS.
